Job summary

SCO Family of Services is seeking a Medical Coordinator/Direct Support Professional in New York. The role collaborates with the RN and Residence Manager to schedule medical appointments, prepare forms, and accompany individuals to appointments as needed.

The position requires organizing medical documentation, maintaining MAR accuracy, and ensuring medications and lab work are up to date, with on-site duties and 25% travel.

Qualifications

• High school diploma or better.
• Valid NYS Driver’s License.
• Strong organizational, interpersonal, and communication skills.

Responsibilities

• Schedule and coordinate medical appointments with RN and Residence Manager.
• Prepare clinical consultation forms for appointments.
• Accompany individuals to medical appointments when possible.
• Maintain and file medical documentation in the individual's medical binder.
• Review and log medications and prescriptions; ensure MAR accuracy.
• Ensure lab work and physician recommendations are up to date.
