Community Options, Inc. in Queens, NY is seeking a Full-Time Medical Coordinator to plan, track, and ensure that all clients' needs are met within appropriate time frames. The Medical Coordinator will schedule annual, follow-up, and emergency medical appointments and communicate with medical staff, supervisors, and program directors.

Qualifications

• High school diploma or GED; bachelor's degree preferred.
• Valid driver's license with a satisfactory driving record.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Ability to exercise good judgment and remain calm in crisis situations.
• Excellent verbal and written communication skills.

Responsibilities

• Schedule all client medical appointments to state mandated licensing regulations.
• Observe and monitor the client's status through paperwork and follow-up appointments.
• Observe and track clients' physical care including annual and emergency appointments.
• Provide accurate, timely, and consistent documentation of client medical appointments in the appropriate records.
• Attend all pertinent medical appointments to address issues clearly and precisely to achieve the best results for the individual.
• Communicate with supervisors, co-workers, and medical professionals regarding clients' status.
• Additional tasks and responsibilities may be assigned.
• Cooperate with license and department staff in inspections, inquiries, or investigations.
