Description

Essential Functions:

  • Prepare consult sheet for each appointment following agency guidelines.
  • Review all consults immediately after appointments, upon return to the facility, regardless of whether the medical coordinator accompanied the consumer.
  • Submit Literal Text changes, new or discontinued orders to ChemRx by the 15th of each month to ensure accurate medication records for the next month’s MAR.
  • Attend weekly Medical Meetings with RN and Manager to review outstanding tasks, diagnostic tests, and appointments.
  • Participate in Clinical Team Meetings:
    • ICF: Quarterly, Annual, and semi-annual as scheduled, or monthly clinical.
    • IRA: Semi-annual, annual, and as needed.
  • Coordinate the Consent for Treatment process, including documentation with RN, manager, and MD.
  • Perform additional tasks related to consumer healthcare or medical systems as deemed necessary by RN or Nurse Administrator.
  • Prepare monthly MAR by reviewing medication inventory daily and ensuring all medication errors are documented and reported.
  • May be assigned other tasks and duties.

  • Perform additional responsibilities as assigned by senior staff, adhering to policies and procedures.
Qualifications

Minimum Job Requirements:

  • Educational: High School diploma or GED; relevant medical certifications and an Associate’s degree preferred.
  • Experience: At least 2 years working with the developmentally disabled, preferably OPWDD population in a residential setting, serving as a Medical liaison and documenting visits.
  • Knowledge, Licenses, etc.: 1-2 years supervisory experience with similar populations; AMAP, SCIP, and CPR certifications required or obtainable within 90 days of employment.

Physical Demands:

  • Clear speech and good hearing.
  • Ability to read data, documents, spreadsheets, and reports in print and digital forms.
  • Ability to provide physical care, including bathing, feeding, diapering, and wheelchair pushing.
  • Ability to physically restrain challenging behaviors.
  • Proper lifting techniques.

Work Environment:

  • Noise levels typical of office, educational, residential, and health environments.
  • Potential hazards consistent with such environments.
  • Work with individuals from childhood to adulthood.
