Salary Range: $19.54 - $23.37 Hourly

Job Shift: Day

Essential Functions
  • Prepare consult sheet for each appointment regardless of locale per agency guidelines.
  • Review all consults after appointment immediately upon return to facility.
  • Submit Literal Text changes, new or discontinued orders to ChemRx by the 15th of each month.
  • Meet weekly for Medical Meeting with RN and Manager to review tasks, diagnostic tests, and appointments.
  • Attend Clinical Team Meetings: ICF quarterly, annual and semi-annual, IRA semi-annual, annual and as needed.
  • Organize consent for treatment process and coordinate documentation with RN, manager, MD.
  • Any additional tasks related to consumer health care or medical system as deemed necessary by RN or Nurse Administrator.
  • Prepare new monthly MAR: daily review of MAR/Medication inventory.
  • Ensure all medication errors are documented on an occurrence report and reported to RN and manager.
  • May be assigned other tasks and duties.

Qualifications
  • Educational: High School diploma or GED. Preferably relevant medical certifications and Associate’s degree.
  • Experience: Minimum 2-year experience working with developmentally disabled population, preferably OPWDD population in a residential setting; serve as medical liaison among physicians, services, IDT team, and family.
  • Specialized Knowledge, Licenses, etc.: 1-2 years solid supervisory experience with similar population. AMAP, SCIP, and CPR certification required; or agree to obtain within 90 days.
  • Physical Demands: Regularly speak clearly and hear; near vision to read documents; provide physical care for clients; restrain challenging behaviors; apply proper lifting techniques.
  • Work Environment: Consistent noise level of an office, education, group home, residential, rehabilitation, industrial, workshop or health related setting. Hazards consistent with those environments. Work with individuals aged from childhood to adulthood.
