Job summary

Visualutions, Inc. seeks a Certified Medical Coder to review clinical documentation and assign codes according to current guidelines and organizational procedures, supporting accurate claims and timely revenue cycle outcomes.

You will validate documentation before coding, escalate gaps when needed, and maintain audit-ready records while using EHR systems to complete coding tasks with clear status updates.

Qualifications

• Active professional coding certification (CPC/CCS or equivalent).
• Working knowledge of medical coding guidelines and documentation standards.
• Familiarity with EHR and coding systems.
• Prior experience in medical coding or revenue cycle operations preferred.

Responsibilities

• Review assigned encounters daily to support complete, accurate coding.
• Assign medical codes in accordance with current coding guidelines and organizational procedures.
• Validate documentation before coding to identify missing, unclear, or conflicting information.
• Escalate documentation gaps, guideline conflicts, or compliance concerns to Team Leads with relevant facts.
• Maintain accurate coding records and supporting documentation for assigned work.
• Use assigned EHR, coding, and workflow systems to complete coding tasks and document work activity.
• Communicate workload status, risks, and questions clearly to Team Leads or Supervisors.
• Participate in coding updates, training, and feedback to stay aligned with current standards and process expectations.
