Job summary

HealthFinders Collaborative in Minnesota is seeking a Medical/Clinical Assistant to support clinicians and care teams in delivering high-quality, patient-centered care. The role involves preparing patients for exams, documenting vital signs and histories, and assisting with procedures.

Responsibilities include coordinating referrals, helping with wraparound services, maintaining a clean and compliant clinical space, and documenting in the EHR.

Qualifications

  • High school diploma or GED.
  • Graduate of an accredited Medical Assistant program.
  • Current Medical Assistant certification or ability to obtain certification within a specified timeframe.
  • Current Basic Life Support (BLS) certification.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work effectively in a team-based healthcare environment.

  • Accurately document patient information in the electronic health record (EHR).
  • Collaborate with providers, nurses, care coordinators, and behavioral health staff to support patient care plans.
  • Assist with referrals, follow-up appointments, diagnostic testing, and specialty care coordination.
  • Help identify and address barriers to care, including transportation, language, insurance, and social service needs.
  • Support patients in accessing wraparound services and community resources.
  • Provide patient education and reinforce provider instructions regarding medications and care plans.
  • Promote understanding of care plans and encourage engagement in healthcare.

HealthFinders Collaborative is a nonprofit community health center dedicated to providing quality, culturally responsive healthcare and holistic wraparound services to underserved communities. Through integrated medical, dental, behavioral health, care coordination, and community support services, HealthFinders strives to improve health outcomes and reduce barriers to care.

The Medical/Clinical Assistant (MA) / Clinical Assistant is an integral member of the patient care team and serves as a liaison between patients, providers, and care coordination staff. This position supports providers in delivering high-quality, patient-centered care while helping ensure patients receive comprehensive services that address their medical, social, and behavioral health needs.

The Medical/Clinical Assistant assists with direct patient care, clinical workflows, patient education, and coordination of services. This role contributes to a welcoming, efficient, and compassionate environment that promotes health equity and whole-person wellness.

Requirements
Essential Duties and Responsibilities
Clinical Support
  • Prepare patients for examinations, procedures, and treatments.
  • Obtain and document vital signs, medical histories, medication lists, and other relevant patient information.
  • Assist providers during examinations and procedures.
  • Perform clinical duties within scope of practice, including specimen collection, point-of-care testing, medication administration, and other delegated tasks.
  • Maintain exam rooms, clinical equipment, and supplies according to infection prevention and safety standards.
  • Accurately document patient information in the electronic health record (EHR).
Care Coordination Support
  • Collaborate closely with providers, nurses, community health workers, care coordinators, and behavioral health staff to support patient care plans.
  • Assist with referrals, follow-up appointments, diagnostic testing, and specialty care coordination.
  • Help identify and address barriers to care, including transportation, language, insurance, and social service needs.
  • Support patients in accessing HealthFinders' wraparound services and community resources.
Patient Engagement and Education
  • Provide patient education and reinforce provider instructions regarding medications, treatments, preventive care, and chronic disease management.
  • Promote patient understanding of care plans and encourage engagement in their healthcare.
  • Foster a welcoming, respectful, and culturally responsive experience for all patients.
Administrative Support
  • Support scheduling, patient flow, and clinic operations.
  • Process clinical forms, prior authorizations, referrals, and other documentation as assigned.
  • Ordering supplies and maintaining inventory list for clinical supplies, insulin, etc.
  • Participate in quality improvement initiatives and organizational performance goals.
  • Maintain confidentiality and compliance with HIPAA and organizational policies.
Qualifications
Required
  • High school diploma or GED.
  • Graduate of an accredited Medical Assistant program or equivalent clinical training.
  • Current Medical Assistant certification or ability to obtain certification within a specified timeframe.
  • Current Basic Life Support (BLS) certification.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work effectively in a team-based healthcare environment.
Preferred
  • Experience in community health, family practice, primary care, or Federally Qualified Health Center (FQHC) settings.
  • Experience working with diverse, multicultural, and underserved populations.
  • Bilingual proficiency in English and Spanish.
