Job summary

CAN Community Health is hiring a Medical Case Manager I in Miami Beach, FL to coordinate and deliver comprehensive case management services, ensuring access to care and support in line with guidelines. You will screen eligibility, develop care plans, link resources, and collaborate with healthcare teams to improve health outcomes.

Qualifications

• Bachelor’s degree in social work, Nursing, Public Health, or related field.
• One year of HIV prevention/intervention experience.
• Must pass Level I and Level II Background checks; valid driver’s license.

Responsibilities

• Screen clients for program eligibility and biannual reassessments.
• Verify insurance coverage and coordinate pre-authorizations.
• Develop, implement and update individualized care plans with patient goals.
• Act as liaison between patients and the care team to address needs.
• Collaborate, monitor, and evaluate the case management process.
• Perform comprehensive assessments of health, mental health, social support, and other areas.
• Evaluate client medical acuity to set priorities and standards of service.
• Coordinate access to primary care and attend medical appointments as needed.
• Submit monthly billing documentation in accordance with grant standards.
• Document client encounters within software within 48–72 hours.
