Qualifications

• High School Diploma required; bachelor's degree preferred.
• 1 year of Care Coordinator experience.
• 2 years of Behavioral Health experience.
• Experience with substance use disorders preferred.
• Strong communication and problem-solving skills.
• Ability to work with a diverse population.
• Computer literacy and EMR system familiarity.

Responsibilities

• Assist clients in accessing ongoing services (medical, dental, vision, housing, financial assistance, legal advocacy).
• Support clients in obtaining or reactivating disability benefits and Medicaid.
• Ensure timely medication distribution and compliance.
• Conduct daily medication checks and document compliance in the EMR system.
• Monitor new or updated medications for documentation and continuity.
• Complete incidental funds paperwork with accuracy and adherence to agency policies.
• Assist counselors with discharge planning.
• Perform additional duties as assigned.

As a Medical Care Coordinator, you are the person who ensures clients make their appointments, get their vital medications, and access critical life resources.
