You will help clients access medical, dental, vision services, housing and financial aid, ensure timely medication distribution, and document in the EMR system.

Collaborating with counselors on discharge planning, you’ll contribute to stability and long‑term recovery in a mission‑driven nonprofit.

Qualifications

• High School Diploma required; bachelor’s degree preferred.
• 1 year of Care Coordinator experience.
• 2 years of Behavioral Health experience.
• Experience working with individuals with substance use disorders preferred.
• Strong communication and problem‑solving skills.
• Ability to work effectively with a diverse population.
• Computer literacy and comfort with EMR systems.

Responsibilities

• Assist clients in accessing ongoing services, including medical, dental, vision, housing, financial assistance, and legal advocacy.
• Support clients in obtaining or reactivating disability benefits, Medicaid, and other needed resources.
• Ensure timely medication distribution and support medication compliance.
• Conduct daily medication checks and document compliance in the EMR system.
• Monitor new or updated medications to ensure appropriate documentation and continuity.
• Complete and upload all incidental funds paperwork with accuracy and adherence to agency policies.
• Assist counselors with discharge planning.
• Perform additional duties as assigned.
