Cosán Group is seeking a compassionate Care Coordinator to support a large patient panel from a fully remote home office in the United States.
You will conduct monthly outreach, provide health coaching, and collaborate with clinical teams to close care gaps using CCM, RPM, and behavioral health integration. A CMA/RMA/CCMA or CPhT certification and EMR experience are required.
This role offers a Monday–Friday schedule with HIPAA-compliant workspace and company-provided equipment.
Care Coordinator (WFH - US)
This is a work from home position, it is not mobile. Candidates must have a private, stationary, and HIPAA-compliant workspace within their home that is dedicated solely to work. Candidates must reside in the United States to be considered.
