Job summary

Qualifications

• Clinical competency verified within 90-day orientation and annually thereafter.
• Excellent customer service and patient interaction skills.
• Basic computer skills preferred.

Responsibilities

• Assist providers with patient examination prep and minor procedures; support patient flow and clerical tasks.
• Participate in daily team huddles and review scheduled visits; monitor patient safety and transitions of care.
• Screen patients for preventive care needs, barriers to care, and potential fall risks; support population health efforts.
