Job summary

Ozarks Community Hospital, Inc. in Springfield, MO is seeking a Medical Assistant to join the Long Term Care Medical Home department. The MA will provide direct and indirect patient care, coordinate Chronic Care Management, and work with providers to monitor patient health and outcomes.

Requirements include a High School diploma or equivalent, completion of a CNA/MA/LPN-LVN program, and current BLS certification at hire or during orientation with renewal as needed.

Ozarks Community Hospital (OCH), located in Springfield, MO, is seeking aMedical Assistant (MA)to join our Long Term Care Medical Home department.

Reports directly to the Director of Medicare Programs. Provides direct and indirect patient care in partnering regional long term care facilities. Provides care that meets the psychosocial, physical, and general aspects of care. Meets the communication needs of patient and family. Provides care that reflects initiative and responsibility indicative of professional expectations, under the scope of licensure. Maintains regulatory requirements, nursing and facility policies, procedures, and standards. Communicates with providers and team members about patient's clinical condition, including results of diagnostic studies and symptomatology. Oversees Chronic Care Management including enrollment, follow-up, documentation, report running and billing procedures.

Requirements:

• High school diploma or equivalent.
