Job summary

Ozarks Community Hospital, Inc. in Springfield, MO is seeking a Medical Assistant (MA) to join our Long Term Care Medical Home department.

The MA provides direct and indirect patient care in partnering regional long-term care facilities, delivering care that meets psychosocial, physical, and general needs under licensure. Responsibilities include communicating patient conditions with providers and team, and overseeing Chronic Care Management enrollment, documentation, report running and billing

Qualifications

• High school diploma or equivalent.
• Successful completion of a Certified Nursing Assistant (CNA), Medical Assistant (MA), or Licensed Practical Nurse (LPN/LVN) program.
• Current BLS certification upon hire or obtained during orientation or next available class, and maintained thereafter.

Responsibilities

• Provide direct and indirect patient care in partnering regional long term care facilities.
• Communicate with providers and team about patient's clinical condition and diagnostic results.
• Oversee Chronic Care Management enrollment, follow-up, documentation, report running and billing procedures.
