Qualifications

Responsibilities

• Facility Engagement: liaison between nursing, sales, wellness, and leadership.
• Patient Engagement: coordinate care between specialists, therapy, home health, and others.
• Front desk duties: scheduling and related tasks.
• Monthly outreach with residents and families to assess needs.
• Connect patients with local social support services.
• Greet patients and prepare them for visits.
• Take vital signs and assist during visits.
• Coordinate after-visit coordination and QA monitoring.
