Job summary

Providence Medical Group Southwest Washington is seeking a Patient Care Coordination Medical Assistant to support primary care patients by centralizing calls, electronic messages, and care coordination activities. You will document in the EHR and facilitate timely communication between patients and care teams.

The role emphasizes excellent patient experiences, continuity of care, and efficient workflow, with escalation of clinical concerns to licensed staff as needed.

Qualifications

• Minimum one year of work experience as a medical assistant including vital signs, injections and medication administration.
• Washington Medical Assistant Certification upon hire.
• Within 30 days of hire: National Provider BLS - American Heart Association.
• 6 months healthcare experience and 6 months medical assistant experience.

Responsibilities

• Manage centralized patient communication via calls and electronic messages.
• Coordinate care activities and document accurately in the EHR.
• Escalate clinical concerns to licensed clinical staff when needed.
• Support ambulatory care team to promote efficient workflows and patient experience.
