Job summary

Chrysalis of Ogden, UT is seeking a dedicated Transportation Assistant to accompany clients to medical appointments and update their medical information, helping individuals increase independence by upholding the Chrysalis Values: Respect, Safety, Fun, Mentoring and Accountability.

Responsibilities include transporting individuals, maintaining confidentiality, coordinating with House Managers, updating medical history in the database, and ensuring punctuality and professional communication.

Qualifications

• High school diploma or GED required.
• Must be at least 21 years old.
• Must pass a criminal background check and LEIE annually.
• Ability to read, speak and write effectively in English.
• Must be able to drive a company vehicle and hold a valid driver’s license.

Responsibilities

• Transport and accompany individuals to medical appointments.
• Maintain punctuality and regular attendance.
• Stay awake, responsive and communicate effectively with individuals.
• Handle aggressive/assaultive individuals with staff support as needed.
• Notify House Managers of medical appointments.
• Maintain and manage the company vehicle.
