Acentra Health is seeking a Clinical Assessor in North Carolina to conduct assessments for Medicaid beneficiaries. This hybrid position involves completing needs-based eligibility determinations for Personal Care Services (PCS) and Community Alternatives Program (CAP) waivers. Ideal candidates should have an RN or LCSW license and at least two years of nursing experience. The role requires travel up to 60 miles radius and offers a compensation range of $77,000 - $80,000 annually.

Qualifications

• Registered Nurse or LCSW license by the applicable state.
• Minimum of two years of nursing experience.
• Requires travel up to a 60-mile radius.
• Experience with community-based individuals needing assistance is preferred.
• Conducting PCS assessments and HCBS waiver assessments is highly preferred.

Responsibilities

• Conduct assessments for PCS eligibility using standardized tools.
• Ensure PCS is provided based on beneficiaries' needs.
• Maintain privacy and dignity of individuals during assessments.
• Participate in mediation and appeal processes as necessary.
