Job summary

VNS Health is seeking a dedicated outreach and enrollment specialist in New York to support prospective members and families with Medicaid enrollment and community resource connections.

The role involves outreach to community organizations, assessing eligibility, and coordinating with MLTC programs while maintaining accurate records and attending trainings. Travel between regions may be required; bilingual candidates are a plus, and a valid driver’s license may be needed.

Qualifications

Responsibilities

• Performs outreach to community agencies to establish relationships and provides access to services.
• Assists clients with Medicaid enrollment and connects them to MLTC programs.
• Screens financial records to assess enrollment eligibility.
• Coordinates with program staff to access entitlements and community resources.
• Maintains records per program requirements and documents services provided.
• Participates in team meetings to ensure eligibility and appropriate services.
• Arranges transportation and appointments for members as needed.
• Participates in training and professional development.
