Pathways to Life, Inc. in Greenville, NC seeks a Tailored Care Manager to support Medicaid members living with serious mental health conditions and substance use disorders.

This is a whole-person, community-based role coordinating behavioral health treatment, medical care, housing, transportation, and benefits into one integrated plan. You will advocate for members, guide them through complex healthcare and social service systems, help with benefits applications, and coordinate crisis

Competenze

• Bachelor's degree in a Human Services field with at least two years of direct experience working with individuals affected by mental health disorders, substance use disorders, intellectual or developmental disabilities, traumatic brain injuries, or related high-need populations
• Bachelor's degree in a non-Human Services field, plus at least four years of the same direct experience described above

Mansioni

• Behavioral Health Care Coordination and care plan development
• Coordinate behavioral health services including outpatient therapy, psychiatric care, substance use treatment, MAT, Peer Support, ACTT, CST, and residential services
• Monitor member engagement and identify gaps or barriers to care
• Support members through transitions after hospitalization, detox, or residential treatment
• Help members access Medicaid-covered services and understand their options
• Advocate for medically necessary services and continuity of care
• Coordinate crisis response and follow-up care
• Coordinate communication among providers, family, hospitals, therapists, and social service agencies
• Complete Medicaid-required documentation and maintain care plans
