Job summary

Coastal Horizons Center, Inc. is seeking a Tailored Plan Care Manager to join our integrated care team in Onslow County, NC, coordinating with Primary Care and Psychiatric Providers to improve client outcomes.

You will develop patient-centered assessments, set care goals, connect clients with primary care, specialists, social services, and ensure smooth transitions from inpatient or recovery programs, including medication reconciliation.

Qualifications

• Bachelor’s degree in a health-related field and 2 years of direct experience with individuals with behavioral health conditions.
• Preferred: 1 year of care management or coordination experience; CCM certification.
• Certification (CCM) or trained in evidence-based care coordination and clinical assessments.

Responsibilities

• Oversee care management activities and ensure adherence to practice standards.
• Develop comprehensive, patient-centered assessments across biopsychosocial needs.
• Coordinate with clients, families and providers to address barriers related to diagnosis, treatment and access to care.
• Implement care plans and transitional discharge planning based on assessments and patient goals.
• Provide education on diagnoses, medications, available resources, prevention and risk factors to promote self-management.
• Maintain documentation in the care management system in line with HIPAA and organizational policies.
• Support after-hours coverage on a rotating schedule and conduct home visits as needed.
• Advocate and liaise with community services and care team members to coordinate services.
