Job summary

Amity Foundation is seeking a Medi-CAL Coordinator to screen individuals for Medi-CAL eligibility and coordinate placements across STOP and CALAIM programs in Los Angeles County. The role supports reentry and housing/resource connections, aligning participants with the appropriate program modalities and services.

Responsibilities include coordinating with Parole Offices, clinicians, and program staff, ensuring documentation in ARMS, and assisting with enrollment and capacity verification.

Qualifications

• Bachelor's degree in social work, psychology, public health, or related field; or Associate's degree.
• And minimum of two years’ experience in client-related field, providing case management and community outreach.
• Equivalent of a high school diploma with three years of administrative/outreach experience in a diverse population.

Responsibilities

• Screen individuals for Medi-CAL eligibility for STOP referrals and placement.
• Generate tailored placement recommendations based on screening and assessment results.
• Coordinate care across medical, behavioral, and social services.
