Job summary

Responsibilities include assessing physiologic and psychosocial health, developing outcomes, planning and implementing care, administering medications, educating patients and families, and discharge planning.

Qualifications

• Graduate from an accredited School of Nursing.
• Current license to practice as a Registered Professional Nurse in New York State.
• BSN degree preferred; enroll within 24 months and complete BSN within five years.

Responsibilities

• Assess physiologic health status and document plan of care.
• Assess psychosocial health status and support patient needs.
• Identify patient outcomes and establish goals.
• Formulate and prioritize plan of care and coordinate resources.
• Implement plan of care, monitor safety, and revise as needed.
• Evaluate care and measure outcomes for improvement.
• Collaborate with care team and physicians in treatment.
• Perform decubritus care and wound care as required.
• Administer medications safely and educate patients.
