Qualifications

• Current Registered Nurse licensure by the State Board of Nursing for Illinois or Michigan (required).
• Current American Heart Association HealthCare Provider BLS is required prior to start date.
• BSN degree preferred.
• Experience as an RN in a healthcare environment preferred.
• Electronic Medical Record and Microsoft Office basic proficiency.

Responsibilities

• Provides care coordination across the continuum and collaborates with the multidisciplinary care team.
• Assesses physical symptoms, provides psychological support to patients, families, and friends, and mentors team members.
