Qualifications

• Ability to delegate appropriately within nursing team.
• Knowledge of discharge planning and post-discharge needs.

Responsibilities

• Educate patients and families on conditions, treatments, and next steps.
• Ensure safe, compassionate care and mitigate risk.
• Document health information accurately in IT systems.
