Responsibilities

• Provide direct patient care using nursing process per scope and standards.
• Perform plan of care interventions including medications and procedures.
• Document care provided and monitor patient condition with the team.
• Assist with activities of daily living and patient education.
• Coordinate transfers, discharges and referrals as needed.
• Advocate for patient safety and well-being.
