Job summary

Appling Healthcare is seeking a Licensed Practical Nurse (LPN) to provide patient care in a hospital setting. You will assess patients at the beginning of each shift and as conditions change, document findings, and inform the RN based on severity.

You will contribute to discharge planning and patient education, with rounds every two hours and medication administration per policy. The role requires a GA LPN license, CPR certification, and a High School Diploma.

Qualifications

• Current GA state LPN License.
• Licensed Practical Nurse Certificate.
• Current CPR Certification.
• High School Diploma.
• LPN education refresh (20 hours every 2 years) as required by board.

Responsibilities

• Assess patients at shift start and as condition changes.
• Document assessments and notify RN promptly based on severity.
• Provide discharge information and ensure understanding.
• Perform patient rounds every two hours and administer medications as per policy.
• Document nursing care including interventions and patient responses.
• Communicate reports at shift start and end for continuity of care.
• Assist in other hospital areas as needed while maintaining PHI confidentiality.
