Job summary

Eden Vista Ripon is seeking a Mem Passer to provide compassionate companionship and assist Memory Care residents with daily activities in Ripon, WI. Join a dedicated team focused on safety, comfort, and well‑being in a warm environment.

You will support mealtimes, memory care duties, and participate in activities designed to help residents stay as independent as possible.

Qualifications

• Experience in long-term care or assisted living.
• Ability to communicate well verbally and in writing.

Responsibilities

• Provide compassionate companionship and support.
• Assist memory care residents with daily activities to promote independence.
• Lead one-on-one and group activities.
• Support residents during meals and assist with memory care laundry and light housekeeping.
• Follow Embrace Memory Care philosophy.
• Document daily per policies and report concerns.
• Ensure safety and train staff on safe practices.
• Address resident, family, and employee concerns promptly.

Eden Vista Ripon is seeking a compassionate and dedicated Med Passer to join our team in Ripon, WI. The ideal candidate will be passionate about providing exceptional care to our residents, ensuring their safety, comfort, and well-being in a warm and supportive environment.

Job Description

• Provide kind, caring, and consistent companionship and support.
• Provide assistance, encouragement, and restorative interventions, as needed, for Memory Care residents to be as independent as possible in their activities of daily living including but not limited to bathing, dressing, grooming, toileting, feeding, and mobility.
• Participate in and lead one on one and group activities.
• Serve residents during mealtime, assist with memory care laundry, and basic housekeeping within the memory care neighborhood.
• Adhere to and is a champion for the Embrace Memory Care philosophy.
• Document daily according to memory care policies and procedures and state regulations, including but not limited to, the communication of any resident concerns, changes in condition, or related topics.
• Ensure that SLC policy and procedure are followed to maintain a safe work environment through the prevention of accidents, the preservation of equipment, and training of staff on safe working practices.
• Ensure that resident/family/employee concerns receive a timely response and follow up, thus striving to provide a high level of customer satisfaction.
• Perform all other duties as assigned.
