Job summary

Eden Vista Hoffman Estates is seeking a Wellness Nurse to provide planning and delivery of direct and indirect patient care. You will supervise nursing assistants and ensure high-quality care in memory-impaired residents.

Responsibilities include medication administration, communicating changes to physicians and families, and maintaining accurate documentation per facility policy. A valid LPN or RN license is required, with long-term care experience preferred.

Qualifications

• You must hold a current State License as LPN or RN.
• Experience in long-term care is preferred.
• Strong knowledge of memory-impaired resident care and medications.

• Plan and deliver direct and indirect patient care using the nursing process.
• Direct day-to-day activities of nursing assistants.
• Prepare and administer medications as ordered by physician.
• Communicate changes in patient condition to physician, supervisor, and family.
• Complete required record keeping on admissions, discharges and transfers.

Position: Wellness Nurse

Job Description

• To provide planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation.
• Direct the day-to-day activities of the nursing assistants
• Prepare and administer medications as ordered by the physician and perform other drug administration.
• Communicate changes in patient clinical condition with physician, nursing supervisor, and family
• Complete required record keeping upon residents admission, discharge, and transfer; chart and document per facility policy

Are you:

• Customer service orientated
• You must have a current LPN (Licensed Practical Nurse or RN (Registered Nurse) license in good standing.
• You have a thorough working knowledge of current care standards and regulations
• You are experienced in the hands-on care of memory-impaired residents.
• You can maintain and update effective service plans and supervise the care staff.
• You have a comprehensive working knowledge of current medication regulations and laws.
• A Licensed Practical Nurse- LPN, LPN Nurse, LVN, New Grad LPN, Nursing student
• Experienced in long-term care preferred
• Able to communicate well in both verbal and written forms
• Committed to interacting professionally and with care toward residents, families, and other staff

Requirements

• You love working with people and handle everyday challenges with a smile!
• Graduate or Future Graduate from an accredited nursing program
• Hold a Current State License as a Licensed Practical Nurse
• Able to communicate well in both verbal and written forms
• Have a minimum of 1 year experience as an LPN in a long term care setting
• A natural problem solver
• Are committed to interacting professionally and with care toward residents, families, and other staff
