Eden Vista Fond Du Lac is seeking a compassionate CBRF Med Passer to support residents in a warm, respectful environment. You will assist with medications, daily living activities, and social engagement under supervision, ensuring safety and dignity for each resident.

The role emphasizes teamwork, excellent communication, and adherence to care plans. Positions are full-time or part-time, in-person, with sign-on bonuses for NOC shifts to reward commitment and reliability.

Qualifications

• CBRF Certified Med Passer.

• Experience in long-term care or assisted living.
• Strong communication skills in spoken and written form.
• Professional and caring interaction with residents, families, and staff.

Responsibilities

• Assist with medication reminders and activities of daily living.

Position: : CBRF Med Passer- $1,000 Sign on Bonus!

• $1000 S/O for FT NOC Shift (Pay out will be $250 at 60 days, $250 at 90 days and $500 at 180 days)
• $500 S/O for PT NOC/PM Shift (Pay out will be $250 at 60 days, $250 at 90 days)

Eden Vista Fond Du Lac is seeking a compassionate and dedicated CBRF Med Passer. The ideal candidate will be passionate about providing exceptional care to our residents, ensuring their safety, comfort, and well‑being in a warm and supportive environment.

Main Responsibilities

• Provides assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living.
• Follows the schedule of resident's needs set out by supervisor.
• Provides emotional and social support to residents.
• Documents daily log of assistance.
• Informs supervisor of any resident issues or concerns.
• Respect and encourage the independence and dignity of the residents.
• Respects residents' confidentiality.
• Is familiar with emergency equipment and procedures.
• Attends all required training, in‑service, and staff meetings.
• Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
• Maintains a positive and professional demeanor toward residents, visitors, families, and co‑workers.
• Performs other duties as assigned.
