Job summary

Right at Home Cleveland West is seeking a compassionate Direct Support Professional (DSP) with an active Med Cert 3 to join our team. You will provide personal care, assist with ADLs, and help clients live safely and independently at home.

Responsibilities include administering medications per care plans, supporting mobility with gait belts and lifts when needed, offering companionship, light housekeeping, meal preparation, and reporting changes in health or condition to the care team.

Qualifications

• Active Med Cert 3 certification (required).
• Experience in direct care or caregiving preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide personal care and support with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
• Administer and manage client medications in accordance with Med Cert 3 authorization and care plans.
• Support clients with mobility needs, including gait belt and Hoyer lift use when required.
• Provide companion care, emotional support, and supervision to ensure client safety.
• Perform light housekeeping, meal preparation, and home support tasks.
• Monitor and report any changes in client condition, behavior, or health status.
• Communicate effectively with clients, families, and the care team.
• Follow all agency policies, documentation standards, and individualized care plans.
