Job summary

Corona Post Acute is seeking an LVN with MDS responsibilities in California. The role supports the Director of MDS in resident assessments, care plans and regulatory compliance, ensuring timely and accurate documentation.

You will contribute to a compassionate, rights-respecting care environment, collaborating with a multidisciplinary team to meet residents' medical, nursing and psychosocial needs.

Qualifications

• 1-3 years' experience providing direct care to multiple patients/residents.
• Nursing degree from accredited college or graduation from LVN program.
• Current valid CPR certification and California LVN licensure.

Responsibilities

• Assists the Director of MDS in development, completion and transmission of resident assessments.
• Supports creation of care plans with measurable objectives and timetables.
• Promotes patient dignity, rights and respectful treatment in all interactions.
• Knows and complies with policies, procedures and state requirements for MDS processes.
• Participates in preliminary and comprehensive resident assessments and plan of care.
• Ensures coordination with involved health professionals for timely assessments.
• Maintains professional image and supports teamwork among staff.

POSITION SUMMARY

Assists the Director of MDS in the development, completion and transmission of the resident assessment in accordance with the state requirements and policies and goals of the facility.

DUTIES AND RESPONSIBILITIES

• Ensures that all residents are treated fairly, with kindness, dignity, and respect, and their rights are protected at all times


• Respects and upholds all residents' personal and property rights whenever carrying out duties, including knocking before entering a resident's room.


• Complies with all company and departmental policies and procedures.


• Participates in development and completion of preliminary and comprehensive resident assessment (MDS) and Care Plan in accordance with current rules, regulations, and guidelines, including the implementation of CAAs and Triggers and Plan of Care.


• Ensures that care plan includes measurable objectives and timetables to meet resident's medical, nursing, mental and psychosocial needs, that appropriate health professionals are involved in assessment, and that schedule of activities is developed as required.

KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS

• 1-3 years' experience providing direct care to multiple patients/residents, one years' previous MDS experience in Skilled Nursing environment preferred


• Nursing degree from accredited college or graduation from approved LVN program


• Must possess current, unencumbered license to practice as an LVN in California.


• Current valid CPR certification


• Knowledge of clinical standards of practice, regulations and reimbursement governing long term care and Medicare participation

Salary Range: LVN $39.50 - 41.00/hr*

*The salary range is the one that Corona Post Acute reasonably expects to pay for this position. The actual salary will depend on several factors (e.g., experience, educations, qualifications, etc.).
