Sanford Health is seeking an RN for MDS coordination and direct resident care. You will lead the RAI processes, assess residents, and ensure care plans remain current. The role involves coordinating care with families and interdisciplinary teams.

Responsibilities include supervising nursing staff on the floor, engaging in care planning, and maintaining high standards of practice in line with state regulations. On-call rotation is part of the schedule for this position.

Qualificações

• RN license required or eligible to practice in MN.
• Bachelor’s Degree in Nursing preferred; RAC-CT preferred or related certification may be valued.

Responsabilidades

• Provide individualized nursing care using the nursing process.
• Coordinate resident assessment and care plans; lead in RAI processes.
• May serve as charge nurse, delegating tasks to licensed/unlicensed staff as appropriate.
