Job summary

Delmar Gardens North is seeking Full‑Time MDS Registered Nurses (RN) with strong leadership, assessment and documentation skills. Missouri RN license and MDS experience in long-term care are required, with MatrixCare EHR familiarity a plus.

Responsibilities include accurate documentation, reimbursement under PDPM and Case Mix, and teamwork with the inter-disciplinary staff. Quality improvement participation is encouraged.

Qualifications

• MDS experience in long-term care is required.
• Missouri RN license is required.
• Proficiency with electronic health record systems; MatrixCare a plus.
• Strong knowledge of Missouri Case Mix and Case Mix System.
• Experience with quality improvement initiatives and audits.
• Excellent organizational, leadership, assessment and documentation skills.

Responsibilities

• Proficiently use electronic health record systems; MatrixCare experience is a plus.
• Ensure thorough documentation supporting level of care.
• Collaborate closely with the inter-disciplinary team.
• Obtain accurate reimbursement for Medicare (PDPM) and Medicaid.
• Participate in quality improvement initiatives and audits.
