Job summary

South Mountain Rehab Center in Boonsboro, Maryland is seeking an MDS Coordinator to manage Minimum Data Set assessments, documentation, and care planning. You’ll coordinate with physicians, families, and the care team to ensure accuracy and timeliness.

You will review medical records, collaborate on patient plans, and help streamline the MDS workflow with PCC familiarity and attention to detail. This is a full-time, on-site role with a supportive team and opportunities for professional growth.

Qualifications

• 1 year of MDS experience is required.
• Flexibility to juggle schedules and tasks.

Responsibilities

• Perform and coordinate MDS assessments following regulatory guidelines.
