Job summary

Courtney Manor is seeking an experienced MDS Nurse to stay clinically involved in patient care while maintaining autonomy and strong organizational skills. Qualifications

• RN with nursing license required.
• Experience with MDS nursing and PPS coordination preferred.
• Strong communication and teamwork abilities essential.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses residents via physical assessment, interview, and chart review.
• Discusses resident care needs with care givers including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews hospital and outside information to inform assessments and care planning.
• Coordinates and initiates significant changes in MDS.
• Maintains PPS board and PPS schedules in absence of Care Management Coordinator.
• Keeps current with AANAC requirements.
