Job summary

Ciena Healthcare in Michigan is seeking an experienced Registered Nurse to work as an MDS nurse, remaining clinically involved in patient care without bedside duties. You’ll manage MDS, CAA’s and care plans with autonomy within regulatory timeframes.

Join a collaborative team and leverage your RAI and PPS knowledge to assess residents, coordinate care with physicians and staff, and participate in PPS meetings.

Qualifications

• Registered Nurse with MDS experience.
• Familiar with RAI and PPS processes.
• Experience coordinating care plans.

Responsibilities

• Completes MDS, CAA’s and care plans within regulatory time frames.
• Assesses residents via physicals, interviews, and chart reviews.
• Communicates care needs with physicians, nursing, social services, therapy, dietary, and activity staff.
• Reviews hospital data and external reports to inform assessments and care planning.
• Coordinates and initiates significant changes to MDS.
• Supports PPS meetings and PPS board for monitoring Medicare days and RUGs.
• Keeps current with AANAC requirements.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications

• Registered Nurse, RN
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an MDS Nurse
