Job summary

Fowler Care Center in Fowler, California, seeks an experienced MDS RN to lead the RAI process from admission to discharge and oversee care planning with the IDT. You will ensure accurate MDS coding and documentation in compliance with federal and state regulations.

Qualifications

• Thorough understanding of the OBRA regulations and Minimum Data Set (MDS 3.0).
• Knowledge of the care planning process and MDS workflow.
• Ability to maintain current MDS status of assigned residents according to state and federal guidelines.
• Strong data entry skills and attention to detail.

Responsibilities

• Ensure decisions on additional MDS assessments based on clinical criteria.
• Coordinate and manage daily stand-up meetings reviewing resident care and ARD dates.

• Maintain current MDS status of residents per guidelines.
• Enter resident data accurately into MDS software.

Job Summary

We are seeking a highly skilled and experienced MDS RN to join our team at Fowler Care Center, a state-of-the-art Skilled Nursing Facility in Fowler, California. As an MDS RN, you will play a critical role in ensuring the timely and accurate completion of the RAI process and care management process from admission to discharge. You will work closely with our interdisciplinary team to assess the needs of our residents, provide guidance on MDS assessments and care plan reviews, and maintain accurate documentation in accordance with federal and state regulations.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess the needs of our residents and provide guidance on MDS assessments and care plan reviews.
• Ensure that the Interdisciplinary team makes decisions for completing or not completing additional MDS assessments based on clinical criteria.
• Assist with coordination and management of the daily stand-up meeting, including review of resident care and setting of the assessment reference date(s).
• Comply with federal and state regulations regarding completion and coordination of the RAI process.
• Monitor MDS and care plan documentation for all residents, ensuring that documentation is present in the medical record to support MDS coding.
• Maintain current MDS status of assigned residents according to state and federal guidelines.
• Maintain frequent and accurate data entry of resident information into appropriate computerized MDS programs.
• Complete accurate coding of the MDS with information obtained via medical record review, observation, and interview with facility staff, residents, and family members.

Requirements
