Responsibilities

• Coordinate MDS/Restorative conferences and care plan reviews.
• Schedule conference dates and maintain attendance records.
• Notify departments and families of conference details.
• Ensure timely updates to MDS and restorative plans.
• Collaborate with therapy and nursing to implement restorative care.
• Supervise restorative nursing staff and coordinate with HR and DON.
