Job summary

Lineage Health Care Consulting seeks a dual-role RN or LPN to serve as MDS Coordinator and Restorative Coordinator in a facility in Illinois. You will lead care planning, MDS completion, and conference facilitation, while also supervising restorative nursing and quarterly rehab assessments.

You will collaborate with nursing, therapy, and administrative staff to ensure timely MDS updates, restorative plans, and staff training.

Qualifications

• Must be proficient in MDS and licensed in Illinois as RN or LPN.
• Experience in long-term care and care planning with restorative nursing.
• Strong communication, leadership, and organizational skills; computer proficiency.

Responsibilities

• Coordinate MDS/Restorative conference and care plan meeting notes.
• Set up and maintain conference schedules and attendance.
• Oversee restorative nursing measures and documentation.
• Interact with therapy staff and interdisciplinary team.
• Attend care planning conferences and QAPI meetings.
