Job summary

Rockport Healthcare Services is seeking an experienced MDS Coordinator (LVN or RN) to join our clinical team in Los Angeles. The role focuses on completing MDS assessments in line with CMS, state requirements, and regulatory standards.

The candidate will coordinate the IDT assessment and care planning process, review broad clinical documentation, and develop care plans tailored to resident needs. Strong organization, communication, and teamwork are essential.

Qualifications

• Active LVN/LPN or RN license in the applicable state.
• Experience in skilled nursing, long-term care, rehabilitation, or related setting preferred.
• Knowledge of CMS/RAI requirements and long-term care regulations preferred.

Responsibilities

• Coordinate and complete MDS assessments per CMS, state, and regulatory requirements.
• Ensure MDS assessments are accurate, timely, and well-documented.
• Coordinate IDT assessment and care planning processes.
• Review clinical records, nursing documentation, therapy notes, physician documentation, and related information.
• Develop and update comprehensive care plans based on assessments.
• Collaborate with nursing, rehabilitation, dietary, social services, physicians, and other IDT members.
• Monitor MDS schedules and ensure timely completion.
• Participate in resident care plan meetings and communicate with residents and families.
• Support documentation and reimbursement for Medicare/Medicaid services.
• Assist with audits, surveys, and regulatory compliance.
• Participate in QAPI initiatives and improve documentation and resident outcomes.
• Educate staff on MDS requirements and documentation standards.

What You’ll Do

• Coordinate and complete MDS assessments in accordance with current CMS, state, and regulatory requirements.

• Ensure MDS assessments are accurate, timely, and supported by appropriate clinical documentation.

• Coordinate the interdisciplinary team (IDT) assessment and care planning process.

• Review clinical records, nursing documentation, therapy notes, physician documentation, and other relevant information.

• Develop and update comprehensive care plans based on resident assessments and identified needs.

• Collaborate closely with nursing, rehabilitation, dietary, social services, activities, physicians, and other members of the IDT.

• Monitor MDS schedules and ensure assessments are completed within required timeframes.

• Participate in resident care plan meetings and communicate effectively with residents and families.

• Support accurate documentation and appropriate reimbursement related to Medicare/Medicaid services.

• Assist with audits, surveys, and regulatory compliance activities.

• Participate in QAPI initiatives and identify opportunities to improve clinical documentation and resident outcomes.

• Provide education and guidance to staff regarding MDS requirements and documentation standards.

What We’re Looking For

• Current, active LVN/LPN or RN licensein the applicable state.

• Previous experience in skilled nursing, long-term care, rehabilitation, or a related healthcare setting.

• Previous MDS experience strongly preferred.

• Knowledge of CMS/RAI requirements and long-term care regulations preferred.

• Strong clinical assessment and documentation skills.

• Excellent organization and attention to detail.

• Ability to meet deadlines and manage multiple priorities.

• Strong communication and teamwork skills.
