Qualifications

• Bachelor's in Nursing required.
• MDS 3.0 experience required.
• Long Term care experience preferred.

Responsibilities

• Complete accurate MDS assessments and CAAs as assigned.
• Monitor and audit clinical records for accuracy and timeliness.
• Coordinate with physicians and discharge planners for referrals and data capture.
• Maintain knowledge of Medicare criteria and RAC-CT certification.
• Monitor Case Mix Index (CMI) for risk and Medicaid impact.
• Perform additional duties as assigned.
