Job summary

Kadima Healthcare is seeking an MDS Nurse (RN/LPN) to join our team as a supervisory minded professional. The role focuses on coordinating and completing resident assessments in line with Pennsylvania requirements and facility goals.

Qualifications

• Graduate of an accredited Nursing program.
• RN or LPN license valid in PA.
• Experience in skilled nursing or acute care preferred.
• MDS experience highly preferred.

Responsibilities

• Interview residents for assessments and coordinate the resident's assessment.
• Evaluate residents' conditions and medical data to determine needs for the assessment.
• Develop and implement care plan with the Director of Nursing to inform the assessment team.
• Schedule resident assessment and care plan meetings with nursing staff.
• Ensure care aligns with the resident's wishes and discharge planning needs.
• Coordinate comprehensive care plans with timelines and measurable objectives.
