Ciena Healthcare is seeking an MDS Nurse to lead the accurate completion of MDS/RAI processes and care planning in Michigan. You will work with the interdisciplinary team to gather data, ensure regulatory compliance, and support PDPM guidelines while maintaining timely assessments and care plans.

Qualifications

• MDS/RAI knowledge and PDPM understanding.
• ICD-10 coding and quality measures familiarity.
• Ability to lead and coordinate interdisciplinary teams.
• Strong regulatory compliance and independent functioning.

Responsibilities

• Coordinates scheduling the RAI process with the interdisciplinary team.
• Completes the MDS, CAAs and care plans within regulatory timeframes.
• Gathers timely data to ensure accurate MDS completion.
• Ensures compliance with PDPM and Case Mix payment systems.
• Communicates changes in resident care needs to the IDT.
• Conducts thorough resident assessments during quarterly reviews.
• Maintains PPS log and coordinates PPS meetings per Medicare schedule.
• Participates in direct/indirect Resident care as directed.
• Coordinates Resident Care Conferences with social worker and families.
• Adheres to safety, risk management, QA, and policy applications.
