• Ensure timely and accurate MDS submission
• Collaborate with the interdisciplinary team to develop and update care plans; collect and validate supporting documentation from Nursing, Rehab, Social Services, Dietary, and Activities
• Monitor documentation to support PDPM and quality measures
• Participate in care plan meetings with residents and families
• Assist with survey readiness and audits
• Educate clinical staff on charting best practices that support accurate MDS coding and QMs
• Review Quality Measures (QMs), 5-Star metrics, and facility dashboards; help drive performance improvement initiatives
