Qualifications

• Current RN or LPN license in the state of Ohio.
• Previous MDS experience preferred (training available for the right candidate).
• Strong knowledge of CMS regulations and RAI process.
• Excellent organizational and time management skills.
• Ability to work collaboratively with a multidisciplinary team.
• Strong communication and documentation skills.

This role is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in accordance with federal and state regulations, ensuring accurate reimbursement, and supporting high-quality resident care.

Key Responsibilities:

• Coordinate and complete MDS assessments (OBRA and PPS) in a timely and accurate manner
