Job summary

Gateway Post Acute is seeking a knowledgeable MDS Nurse (LVN or RN) to join our interdisciplinary team. This full-time role focuses on accurate resident assessments, regulatory compliance, and high-quality care.

Compensation ranges: LVN $72,000-$78,000; RN $82,000-$100,000 annually. Pay is based on experience and qualifications. We value detailed documentation and collaboration with the care team.

Qualifications

• California LVN or RN license currently active.
• Experience in skilled nursing, long-term care, or post-acute care preferred.
• MDS and care planning knowledge is a plus.
• Familiarity with Medicare/Medicaid, PDPM and regulatory requirements beneficial.

Responsibilities

• Coordinate and complete accurate MDS assessments within required timeframes.
• Collect, review, and verify resident clinical information.
• Ensure MDS documentation complies with federal, state, and facility requirements.
• Monitor assessment schedules and maintain timely completion of all required documentation.
• Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.
• Participate in care plan meetings and assist with development of individualized resident care plans.
• Review medical records to ensure documentation supports resident needs, services, and reimbursement.
• Identify documentation concerns and provide education to clinical staff as needed.
• Maintain the confidentiality of all resident and facility information.
• Support quality assurance and performance improvement initiatives.
