Job summary

Woodlands Senior Care Centers is seeking an experienced MDS Nurse (RN or LPN) to coordinate and complete MDS assessments in accordance with CMS requirements. This role partners with the interdisciplinary team to ensure timely documentation and quality care in a long-term care setting.

Pay is $33.00–$40.00 per hour with generous benefits including paid time off, flexible scheduling, health options, 401k, and opportunities for advancement within the company.

Qualifications

• Current Ohio RN license, preferred.
• MDS experience preferred.
• Long-term care/SNF experience preferred.
• Strong organizational, documentation, and communication skills.

Responsibilities

• Coordinate and complete MDS assessments in accordance with CMS and regulatory requirements.
• Lead the interdisciplinary team through the MDS and care-planning process.
• Ensure accurate and timely clinical documentation.
• Oversee and coordinate the Restorative Nursing Program.
• Educate and support nursing staff regarding restorative programs.
• Assist with survey readiness and quality improvement initiatives.
