Job summary

Austinwoods Nursing Center is seeking an experienced RN or LPN to join our MDS program in Austintown Township, Ohio. The role focuses on coordinating MDS assessments, leading the interdisciplinary team, and ensuring accurate clinical documentation.

Qualified candidates should hold an Ohio RN license (preferred), have MDS experience and long-term care/SNF background, and possess strong organizational and communication skills. Pay is $33.00–$40.00 per hour with generous benefits.

Responsibilities

• Coordinate and complete MDS assessments in accordance with CMS and regulatory requirements.

• Oversee and coordinate the Restorative Nursing Program.
• Educate and support nursing staff regarding restorative programs.
• Assist with survey readiness and quality improvement initiatives.
