Job summary

The Advocate Healthcare Center of East Boston is hiring an MDS Coordinator to manage MDS assessments, coordinate care planning with interdisciplinary teams, and ensure regulatory timelines are met.

Responsibilities include data analysis for quality improvement and reimbursement, staff education on documentation standards, and collaboration across nursing, therapy, and administrative teams. MA licensure required with relevant long-term care experience.

Qualifications

• Current RN or LPN license in MA.
• Experience in long-term care or skilled nursing facility (1–2 years).
• MDS 3.0 knowledge is highly desirable.
• RAC-CT certification preferred or willingness to obtain.
• Strong understanding of Medicare and Medicaid reimbursement processes.
• Exceptional attention to detail, organization, and communication skills.
• Ability to work collaboratively in a team.

Responsibilities

• Complete and submit accurate and timely MDS assessments per federal and state regulations.
• Coordinate interdisciplinary team meetings to develop individualized resident care plans.
• Monitor and track assessment schedules to ensure regulatory timelines.
• Analyze MDS data to support quality improvement and reimbursement optimization.
• Educate staff on care documentation standards and regulatory changes.
• Collaborate with nursing, therapy, and administrative teams to ensure resident-centered care.
• Participate in audits, surveys, and inspections as needed.
