Job summary

The Laurels of Bedford is seeking an experienced Care Management Nurse, MDS Nurse to engage in the RAI process and coordinate care plans for residents. You will work autonomously, manage assessments, and collaborate with the care team to ensure timely MDS documentation.

Requirements include RN or LPN licensure and knowledge of RAI/PPS processes. The role offers benefits and opportunities in a team-focused environment.

Qualifications

• RN or LPN license required.
• Experience as an MDS Nurse preferred.
• Knowledge of the RAI process and PPS strongly preferred.

Responsibilities

• Completes the MDS, CAA's and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS's.
• Conducts PPS meetings and maintains PPS board for Medicare days and RUGs utilization.
• Remains current with AANAC requirements.
